As a HPT Case Manager, your key responsibility is to support 18 to 24-year-old clients in their journey toward work readiness, sustainable employment, and independence. You will empower Taitamariki by helping them navigate available choices and connect to the right services, all while ensuring they receive the support they need.

 

This role involves working closely with young people at risk of long-term unemployment and poor social outcomes, equipping them with the tools for success in education, training, or employment.

 

About the Service | Mō te Ratonga

 

He Poutama Taitamariki (HPT) is a regional initiative supporting young people at risk of long-term unemployment. HPT provides intensive support through training and employment opportunities, building on pilots like KODE, GROW, and T500. This service combines case management with long-term care (manaaki tangata), significantly reducing unemployment and disengagement in Northland since its 2016 launch, benefiting over 2,600 Taitamariki.

•    Client-Centered: You’ll put your clients first, listening deeply, responding with care, and helping them move forward on their unique journeys.
•    Adaptable & Steady: Comfortable with change and navigating through challenges while working in a high-pressure environment.
•    Technologically Confident: You should be comfortable with multiple systems and help Taitamariki do the same.
•    Empathetic and Professional: You’ll need to apply policy with empathy, understanding that real-life challenges require practical solutions.
•    Detail-Oriented: You must be fast and accurate with data entry and comfortable with numbers, handling calculations related to payments and entitlements.
•    Clear Communicator: You’ll build trust through clear communication and decision-making.
•    Team Player: Grounded, dependable, and collaborative in a supportive environment.
